Research project: Assessing the performance of knee athroplasty using clinical testing, motion analysis, electromyography (EMG) and musculoskeletal modelling - completed

Patients who have undergone knee replacement have poorer function compared to healthy people of similar age. There are many factors that could contribute to this loss of function but there has been no research looking at multiple factors pre- and post- knee replacement. This project aims to use the latest clinical testing, motion analysis, and musculoskeletal modelling to assess patients’ function. A hierarchy of factors that affect function will be produced from this project to shed light on the reasons for reduced function provide a focus for future research.

Knee replacement surgery is commonly performed to reduce pain and increase function in individuals with degenerative joint disease. In 2008 over 70,000 operations were performed in England and Wales, with a success rate of over 90%. However there is known reduction of function compared to healthy age matched individuals. Current research into this loss of function has focused on single factors, for example muscle strength. The problem with this type of research is that there are many factors that can affect function and there is a need for a more comprehensive assessment.

This project aims to collect data from the patients both pre- and post-operation on anthropometrics, strength, range of motion, proprioception, pain, stability, and activities of daily living. Three-dimensional motion analysis is also being used to assess patients during gait, sit-stand-sit, and step-descent to gather information about joint kinematics. This motion capture and force plate data will then be imported into AnyBody Musculoskeletal modelling system to build inverse dynamic simulations of the patients movements. All data will then be classified and reduced using statistical methods to provide a hierarchy of factors which affect both subjective (perceived) and objective (observed) function.
