We have two Case Report Forms (CRFs), CRF 1 for the day of recruitment collecting baseline information and demographics and CRF 2 for completion if a child is aged 6years + and has a peak flow test undertaken at day 28 post recruitment.
All forms should be in the participant pack you have been provided with. If you use the paper form and do not enter directly onto the online database, please be sure to add a PID numerical sticker from the pack and transfer as soon as you are able particularly if the child is recruited to the trial so we have a record of the medicine ID in case of the need for unblinding.
CRF 2 needs adding to the database too.
