Radiology request forms

The radiology forms in the PID pack are for sending (in addition to your usual request form) to request the image be taken. Please note these need to be signed by someone who has undertaken IRMER training (probably a doctor or nurse practitioner). Please complete the top section fully, the radiology department will be expecting children. 

The second form - confirmation of request - is sent to your Centre (University) to ensure you are paid for the effort you make from the Research Costs payable.
