Practices can identify potential participants in two ways:
1. Opportunistically by the GP or Nurse Practitioner during consultation, taking approximately 20 minutes.
2. Monthly mail out letters of invitation from a search of the practice database taking approximately 1.5 hours.
GPs and Nurse Practitioners will be asked to:
1. Ask eligible patients to give informed consent
2. Collect clinical information using a standardised internet-based form (CRF)
3. Ask patients to complete a web based questionnaire at home (or provide paper copy & SAE if preferable)
4. Ask willing patients to provide blood or saliva samples (including for genetic analysis)
5. Fax paperwork to the research team
6. Two years after recruitment we will also ask participating practices to perform a notes review of about 15 minutes duration for each patient recruited.
