· Additional Exam Recommendations (AERs) only request form.
· Further support (including AERs) request form.
You only need to complete one form.
Your form will be reviewed by our team and we will contact you regarding the specialist support available to you. If a student support appointment is arranged for you, you are welcome to bring your supporter, such as a parent or guardian, with you to join the session.
Please complete this form if you are only interested in accessing AERs, such as extra time in your examinations and if you do not currently require any additional support for your studies or accommodation.
Please be aware that to implement AERs, we require documentary evidence of your condition, which must contain a diagnosis. Before you complete this form, please send your evidence to the Student Disability and Inclusion Team via The Student Hub.
