Transition Programme: request specialist support

On this page you will find two forms:

· Additional Exam Recommendations (AERs) only request form.
· Further support (including AERs) request form.

You only need to complete one form.

Your form will be reviewed by our team and we will contact you regarding the specialist support available to you. If a student support appointment is arranged for you, you are welcome to bring your supporter, such as a parent or guardian, with you to join the session.

 Additional Exam Recommendations (AERs) only request form

Please complete this form if you are only interested in accessing AERs, such as extra time in your examinations and if you do not currently require any additional support for your studies or accommodation.

Please be aware that to implement AERs, we require documentary evidence of your condition, which must contain a diagnosis. Before you complete this form, please send your evidence to the Student Disability and Inclusion Team via The Student Hub.

Further support (including AERs) request form

If you are interested in further support available through our team (as well as or instead of AERs), and feel you would benefit from a discussion around your disability and how this may affect your time at University, please complete this form. 

Following completion of this form, you may be invited to discuss your needs in a 1:1 appointment with a specialist practitioner.

Further support may include:

• Access to the Clarkson Rest and Study Room.
• Learning support assistants.
• Inclusive Support Ambassadors.
• Short-term specialist mentoring.
• Accommodation allowance.

All students accessing support with Student Disability and Inclusion

Whether you only request AERs or request further support, all disabled and neurodivergent students and students with a specific learning difference can access the following support through Student Disability and Inclusion:

• Specialist study skills support.
• Access to specialist assistive technology software. 

 

Do I need to provide evidence of my condition?

If you have documentary evidence confirming your diagnosis (for example, a letter from your doctor/consultant or a diagnostic assessment report), we encourage you to send this to our team via studenthub@soton.ac.uk so that it can be reviewed by our team.

With documentary evidence, we can discuss certain types of support, such as:

• Individual reasonable adjustments, that can be recorded through Student Support Recommendations (SSRs) and shared with relevant teams across the University.
• Additional Examination Recommendations (AERs).

In some cases, detailed evidence from your school or college can be used to put in place temporary reasonable adjustments, such as AERs, so please do send any evidence you may have to our team.

Please do not worry if you do not currently have documentary evidence of your condition. There is a variety of support that may be available to you, with or without evidence, including: 

• Specialist study skills support.
• Access to assistive technology software and spaces.
• Inclusive Support Ambassadors.
• Learning support assistants. 
• Short-term, specialist mentoring.
• Access to wellbeing support.
