Patient Experiences of New Models of Care.

Wessex AHSN, the Centre for Implementation Science at the University of Southampton, and R-Outcomes have produced a report discussing their evaluations of new models of care, including the experience of patients.

A recent focus of work within the Wessex AHSN and Wessex CIS has been the evaluation of two Primary and Acute Care System (PACS) Vanguards: North East Hampshire and Farnham and the Isle of Wight. The report explains the various elements of the mixed-methods approach to evaluation that has been used and how our findings have helped improve new care models such as Integrated Care Teams, Integrated Locality Services, the Enhanced Recovery Service at Home and the Pre-Diabetes Education Programme.
