Absence Reporting Form

Use this form or call the Absence Reporting Line (023 8059 7111) to notify the University of ANY absence from the workplace (except annual leave and working from home*):

Entitlements

Please see the COVID-19 Absence Summary for information on your entitlements to paid time off.

Casual workers should contact UniWorkforce for information about their entitlements.

Informing Line Manager

This form doesn't replace the requirement to notify your line manager of the absence and you still need to call them (or an appropriate alternative person) to let them know you will not be at work.

If you have any questions about your entitlement to paid leave, please contact Ask HR (telephone 023 8059 7547 or email ask_hr@soton.ac.uk).

*Annual leave should still be recorded via MyHR Dashboard. Working from home does not need to be reported via this form or the Absence Reporting Line, unless you are self-isolating.
