Can I have the READ codes for the database search and mailout?

We are recruiting via GP or nurse consultations only at the moment. Once the READ codes are ready (this will be a MIQUEST query), we will send these out to all recruiting practices to start the monthly mail-outs.

Which form goes where?

Paperwork	Send to	Why?
White consent form & Sample Requisition Form (SRF)	The study team in A5 freepost envelope.	Patient data can only be accessed once the consent form has been received by the study team.  We need the SRF so we know to pay you for the samples!
Pink consent form & Trial Request Form (TRF)	The Tissue Bank in the sample pack.	The Tissue Bank won't process the sample without the consent form. The TRF tells the Tissue Bank what to do with the samples when they get them.
