Project overview
Background. Recent findings from our NIHR ARC Wessex funded project, demonstrated that Social Prescribing Link Workers (SPLW) support range in diversity, intensity and frequency, with multi-faceted benefits for patients and the wider community. However, evidently there is a lack of standardisation in approaches for SPLW implementation interventions with pathway gaps and unclear guidance about how to better support and provide structured assessment and longer-term management for people living with physical and mental health (LTCs), particularly those in underserved communities. Thus, guided by the need, it is necessary to develop an SPLW framework that offers a comprehensive pathway for support and can integrate holistic patients’ mental and physical health needs.
The aim of this study is to develop a consistent SPLW framework that sets out sustainable and integrated pathway for optimising social prescribing services in the community, standardises guidance for training support and is equipped to support the long-term management of complex needs of adults with physical and mental health LTCs.
